Once it attaches to the virus or bacteria it either renders it inactive or acts as a tag for other immune system components to attack. The structure the antibody attaches to is specific to the disease - other diseases will usually have different structures so the antibody cannot fit exactly to them and bind. Sometimes related diseases will share a similar structure so that immunizing against one disease will protect against another - for example cow pox was used as a vaccine to help people develop immunity against the different forms of smallpox.
