In GTA IV, Roman's fate is decided on whether you play the choice of missions: Revenge or Deal. If you play Revenge, then Roman will be spared, although Niko will pay another terrible price. If you play Deal, then you have played into Dimitri's hands. So because of Dimitri's hatred of Niko and their past history, this will seal Roman's fate.

∙ 12y agoShe does die, but only if you choose to decline the deal that Jimmy Pegorino and Dimitri Rascolov set up. If you kill Dimitri, Pegorino will show up at your cousin's wedding and shoot Kate with an AK-47. If you schoose to go along with the deal that Jimmy P and Dimitri set up, Kate will only leave you but your cousin will be killed. Either way you loose something.

It depends which ending you choose. Both have devastating consequences for Niko.
If he chooses Deal, then Roman will be shot accidentally just after his wedding, by a hitman hired by Dimitri, trying to kill Niko.
If he chooses Revenge, then Kate will be killed in similar circumstances, by Pegorino doing a drive-by shooting to kill Niko.

∙ 13y agoRoman Bellic will die if you chose the Deal ending. If you chose the Revenge ending, Kate McReary (sister of Packie, Gerald and Derrick McReary). Either way, Dimitri Rascalov and Jimmy Pegorino are killed.
