The site has a set of guidelines that are followed to make things easier and more clear for everyone. Sometimes question wording is changed to the standards of the guidelines, and sometimes it is changed in order to merge the question with another one that is asking for the same information.

Because it is a wiki and anyone can provide an answer. Sometimes they provide answers which are wrong because they do not know the right answer. Sometimes there is an element of opinion in the question and so there is more than one possible perspective which can be addressed in the answer.
