It means you better take it to the VET, and is you cant afford a 20-30 dollar vet visit take it to and aspca they will treat it and find it a good home where as city shelters usually kill after 30 days and dont treat. But you may be lucky and have a no kill shelter in your area. you could also put dog in a portable kennel and drop it off at a vet, BEFORE they open most vets will treat the dog and then find it a home.
your dog could be infested with fleas causing excessive scratch-itching and biting. easy fix a flea bath and advantix applied to skin behind neck. skinny-lack of food or worms, feed it and get a 4-5 in 1 wormer from local petco/smart or Walmart/kmart give it he should crap out dead worms if its worms within 24 hours.
if you gave more info it may have been easier to give right answer. how long you had dog. did you find stray, how old etc.
