The heart is shaped somewhat like a pyramid. The left ventricle, at the "bottom" is the apex, and the "top" is more like the base of the pyramid (even though it is a rather inverted pyramid).
The cardial apex means the apex of the heart. This is the "pointed" end or the tip. The top is called the base.

The upper most anterior / inferior connecting part of the Heart to the Arteries.
The top of the heart is called the apex. The left and right ventricles are located in the apex of the heart.
The wide superior part of the heart is called the base. It is the top of the heart where the major blood vessels enter and exit.
