I'm not sure, but maybe you're filling it up with apps, ringtones, photos, etc; maybe there's something wrong with it; or something. The same thing happened to me on my digital camera memory cards if there's something wrong with it. Maybe it got wet, virus, damaged, some sort of serious problem that mysteriously formed after it started having some malfunctions or getting old. I don't know.
