Really depends on what you mean by "extremely fast". Decent riders can maintain a cadence - pedaling pace, of 100-120 turns-per-minute. For them, it's be a good way of tackling a very steep climb. The bike would be going slowly but their feet would be going fast.

For an average Joe, it'd probably look like they were pedaling real fast. but to a cyclist real fast would mean above 130.

Why has to do with how muscles work. Light and fairly fast is better for endurance than slow and hard.

What the difference between 17 speed and 18 speed transmission?

The difference would be that the 18 speed has one more gear to chose from than the 17 speed. But while 18 speed is easily doable on a bicycle I can't come up with a way to get a bike with 17 speeds - unless you're counting like this:" when I am using this gear I still have 17 others to chose from."
