Depending on the type of illness or injury either just the uterus is removed, or the uterus, fallopian tubes and ovaries are all removed.

When the quality of your life deteriorates due to pain, long periods, or other similar reasons, your doctor will recommend one. You can try simpler alternatives if you are still wanting to have children ( D & C, oblation) but those are not particularly long lasting fixes.
