The famous artist Roberet Rauschenberg made a marvelous series of artworks called "Combines"; some of these are collages. His Combine Painting (1955) is at the Museum of Modern Art in New York.
They made art for religion ceremony's, "good luck" charms, and to demonstrate wealth and to appease anger of the gods.
A sculpture is the actual piece of art that's been made, and the sculptor is person who made it.

Deciding whether a work of art is "successful" is quite subjective. However, here are some questions to consider that can help one determine whether the piece of art is good/successful: 1. Is it original and unlike other works of art that you have seen? 2. Is it representative of the time in which it was made? 3. Is it "fresh"? 4. Does it have meaning? 5. Does it master the medium in which it has been made?
Size (how big is the piece), substance (what medium is it made from: watercolor, oil, pastel, charcoal, pen and ink...), signature (who did the piece) and subject, (what is the piece of).
Where and when the piece of art was made are two very important facts to know. The medium of the piece is also valuable to know.

Descriptive Art is a recently (2012) established art form which purports to take conceptual art to it's logical conclusion, in as much as the 'pieces' are nothing more than written descriptions of a potentially real piece of art - however the intention is clear in descriptive art that the piece should not be made, the assumption is always that the piece will be more complete if left to the readers imagination, in the same way that a novel is usually better read than when turned into a film.
