His Doctor Who was worried about him prescribed him alot of stuff to try at once. Which caused the overdose because the doctor did not know that the drugs were as strong as they were. Basically, Ledger consulted several different doctors about several different medical issues he was having. These doctors unknowingly prescribed medications that shouldn't have been combined, and this is why Ledger wasn't warned. It was notsimply because of one doctor's incompetence, but because several doctors were involved.
