There are almost too many reasons to type that would drive a flyer to eject.
uncontrolled fire, loss of electrical or hydraulic power, loss of engine, impact with ground immenent, overwhelming smoke/fumes in the cockpit, catastrophic systems failure, aircraft got shot/hit by AAA or SAM, etc, etc, etc.
The pilot hit the eject button which launched him out of the airplane.

best take it to a gunsmith. could be several reasons for failure to eject. gunsmith would find the problem.....................
