I would say suffocation of any kind, such as drowning, being smothered, or being strangled. That would be frightening because all you can do is wait there in complete pain for death to come, knowing full well what's happening. You feel every effect it has on your body until you finally pass out. I would also say that being burned to death would be terrifying. You can feel every single thing, and it would be the most painful way to die.
The speed that Bubonic Plague overtook its victims and the gruesomeness of the death were particularly frightening, especially when the cause was unknown.
They could destroy all civilization and infrastructure and essentially wipe out the planet and things would be contaminated for centuries- this does not occur with conventional or limited warfare.

If there were any truth to this superstition today, the profusion of birds flying into skyscrapers would be quite frightening.
