Dumbledore is really, really dead - it was not faked or something like that. Like other dead headmasters, there is a painting of him; while it talks, it is more like a "photocopy" of his character, not Dumbledore himself.
Yes. Harry goes to Voldemort because after viewing memories of Dumbledore and Severus, he finds out Dumbledore planned for him to die, and for Voldemort to kill him. Once Voldemort kills Harry, Harry is given the choice to die or come back and fight. Harry decides to go back and fight, which seems as though he died and then came to life.

We see him again but he isn't alive. He meets Harry in a sort of limbo. Harry can chose to die or go back (because he isn't fully dead only the part of Voldemort inside him is) and Dumbledore talks to him there. Harry asks if it is all in his head and Dumbledore answers: "Of course it's happening inside your head, Harry, but why on earth should that mean that it is not real?"
Harry Potter does die in the last film because he had part of Voldemorts soul inside of him and if he wanted to destroy Voldemort he had to die since he was a horcrux that Voldemort did not mean to make. But when he dies he talks to Dumbledore and Dumbledore says that he can make the decision to go back or to die and Harry decides to go back and kill Voldemort so he does die but he goes back to life.

In "Harry Potter and the Deathly Hallows", Lord Voldemort threatens Hogwarts that if they do not hand over Harry he and his Death Eaters will kill everyone in Hogwarts. Harry goes to confront Voldemort and is killed. Then he finds himself with Dumbledore at King's Cross Station. Dumbledore tells him to go back. When he is back in the real world, he pretends to be dead until he can kill Voldemort. He uses The Elder Wand to slay him. After that he repairs his wand (it had been broken) with The Elder Wand and then he buried The Elder Wand at Dumbledore's grave.
