Considering that lancing a boil is technically surgery, I'd say any time the treatment proceeds below the derma. Note that, with boils at least, you're likely releasing an infective substance, so the path that substance takes upon release -- out of the body and not further in -- is critical. Any time that the problem of post-treatment sepsis cannot be assured without invasive measures, surgery is required.You should understand that when it becomes painful enough, you should go for the surgery to get the relief from this pain.
