Ethical Hacking means you have been given permission buy a company to come in and see if you can find any holes in there security. Its called a security audit. Security consulting firms do it normally for very large companies who do not want their trade secrets to be exposed.
In the modern slang, it basically means the same thing. Sometimes cracking is used to describe illegal decompilation (hacking) of software.

Hacking is a broad term. Cracking, which is gaining access to other peoples computers or accounts, is considered illegal. Hacking simply means using computers to do things most people don't realize they can do, which is not illegal in most cases.
CEH most commonly means Certified Ethical Hacker. It is a term used for people who are employed by organizations to test their cyber security and computer systems by "hacking" them.

For me reading that I would say difference is; a pen-test has a single goal (trophy) and strict procedures that have to be followed to get that trophy, and an ethical hack is a much larger beast that involves many goals or trophies, could last so long it needs to be time restricted, and in general has less limits. So a pen-test is "one goal, one process" and ethical hacking is "hack everything that can be hacked... ...ethically"
