4) case# 1001907 failure/reinstatement date 3/30/2010
Your Honor I also know I did fail to appear in front of you. at the time I got all this tickets I was mixed up with a bad group of people I was on drugs pills & herion after the last ticket I moved out of state Utah with my mother I did I little better until the girl that came with found them same kind of people drugies out here.
So your Honor to basically tell you the truth am in a federal jail awaiting my fate on what the federal Judge is going to throw at me I am looking at about 5-10yrs they say.While I am in here am working on getting a buliders license & my drivers licenses back.
Your Honor I have learened a very hard lesson in life & I am going to learn from all my mistakes I've made.
I aploize for not appearing in your courtroom that just shows no respect and I am truthly sorry.
Please is there anyway you will dismiss these charges agaisnt me & reinstate my license. So when I do get out of here I can get my Utah driver license.

The result of this example would be a driver's license for Mr. Smith which might look like this: S1234-xxxx9-10115 where the x stands for numbers which might be randomly assigned. If I can ascertain if the x actually designates something, I will be back to improve this answer.
