Yes, the loose spoke nipple protrudes into the protective tube tape and could wear a hole in the tape as well as the tube itself. The problem could be woursened if the nipple is very loose and the tube or tire pressure is over inflated. The fix would be prevention of the nipple loosening. Another problem could be when a nipple is too tight and the spoke protrudes out of the end of the nipple and wears a hole in the tube tape and tube. You might get lucky by doing this: deflate the tire, tighten the spoke nipple then inflate the tire to proper psi, then spit on your finger and place the saliva onto the tube and schrader valve: wait and see if bubbles are produced if so tighten the schrader valve and you are set to go.
If there are any small cuts or abrasions on the nipple, or if milk is sucked from the nipple, the aids virus could be transmitted.

The description "ball" is somewhat vague in this context; if you are overweight you could simply have a fat deposit on your chest, or if you are muscular it could simply be your pectoral muscle; it is also possible that you have developed a cyst or a tumor (in which case you need medical attention). If this ball is larger under one nipple than the ball under the other nipple, that would suggest a medical problem.
It could be your period or an infection if you are female. If you are a guy, it could an injury or infection to your urethra.

A crust around the nipple could be a sign of infection, eczema, or other skin condition. Very rarely, nipple changes can be a sign of cancer. See your health care provider for accurate diagnosis.
A retracted or inverted nipple is one that doesn't stick out or protrude past the level of the surrounding skin. If the nipple has always been this way, there is no cause for concern -- it's normal. If a nipple that used to protrude now is retracted, the finding could be a sign of a mass or tumor in the breast.
