File a Form 1040X (an amended return). A suprisingly easy form to complete...while it may look large, only a very few lines normally need to be completed...new amount, old amount, difference and totals.

If you took the amount as a deduction as State taxes on your federal return originally (say refund is from a prior year), then getting it back now is reported as income.
