A lot has to do with your age, because the likelihood that you may need help goes up as you near and pass the end of puberty. If you are not yet out of puberty, you might want advice from you dad or male caregiver if there is one in the picture for you. Mentioning it to your doctor is ALWAYS appropriate. If you can't pull the foreskin all the way back, is it possible when soft to gently push the penis with your finger further down into the foreskin? This might at least give you an opportunity to get warm water and mild soap down in there to cleanse the skin.
If you do need help, don't just jump to the option of circumcision. Circumcision may be the right treatment, but there may be other things that you and your doctor can try before taking that solution.

If you think it maybe more than just smegma, i.e. if you're itching, painful, and there is a bad odor, it might be thrush. Thrush is a fungus. It can be sexually transmitted or caused by poor hygiene and wearing tight underwear or underwear made of unnatural fibers. You can get an over the counter cream for this. It's easy to fix. Or you could always go to a clinic/doctor and ask them for help/advice.
