No. Most payments must be made electronically using EFTPS, but you may still file paper forms.
Yes, you can fax Form 941 to the IRS, but only if you are submitting it for specific reasons, such as to correct an error or respond to an IRS notice. The IRS has designated fax numbers for different forms and situations, so it's important to verify the correct number on the IRS website or the form instructions. For regular submissions, it is generally recommended to file Form 941 electronically or by mail.

Yes. Form 941 is Employer's Quarterly Federal Tax Return. It's filed quarterly [i.e., every three months] even if you have no taxes to report.
This is easy. Go to the IRS website, irs.gov. and on the main page there is a section for Forms and Publications. You will simply go to previous years forms and publications. When the menu comes up on the next page, put the number 941 in the form lookup. It will list every past year in order for the form. You simply click on it and it will download and you can then print the form.
There are several addresses; please see the link below.

Only employers have, or actually send the IRS, a Form 941 - quarterly. There is no such thing with an employees name on it. It is on a company wide basis and relevant to payroll periods and deposits made through that time. Frequently, it is required to be electronically filed and hardcopy isn't much more than a receipt. If your an employer needing a copy contact your payroll service center...although don't be suprised that it may take a very long time to get it. Although youshuld be able to produce whatever info you provided from your payroll records easier.
