"ACH" = "Automated Clearing House" refers to credits or debits made via your bank or other financial institution (e.g., savings account, checking, credit card, debit card, etc), which, presumably, you authorized (e.g., automatic payments to your credit card company, utility company, online retailer, etc)

"IVR" = "Interactive Voice Recognition" refers to payments made via a phone system with a human being or an automated (e.g., "Press 1 to make a payment," etc) system. People pay by phone when they are close to the payment deadline and forgot to make the payment, or didn't have enough in the bank to cover it at the time, or changed bank accounts, or wanted to pay in advance, or any of a number of similar reasons.

"VISB" = is probably a VISA card payment, which could be a credit card, debit card, gift card, etc. (I am not 100% sure that that is what VISB means; I could not find it anywhere on the web, except that there are many such "VISx" listings with different alphabet letters replacing the 'x')

Together, it seems that you made a payment to some company via the phone using VISA. The name of the company would be shown somewhere on the same line as the "ACHIVR VISB" or underneath it, etc. FWIW, there are lots of such bizarre and arcane listings on bank and financial statements. It is, sometimes, best to ignore such, and far more important to look at the name of the company and the credited/debited amount, in order to make sure that it is legitimate. Unless this is a new charge on your bill, you may find something similar for the same company on a previous bill, but, for whatever reason, you didn't notice it.
