To change the injectors on a 6.0 you must first
1. Remove the valve covers
2. Remove the high pressure oil rail. Be careful not to drop any bolts, not to break off the injector connectors, they break easily.
3. Remove the injector clips on the outside of the head, inboard towards the turbo.
4. unbolt the injector
5. Use an injector puller so that you dont damage the injector, or injector bore.

You could have bad injectors or glow plugs. It is hard to say without more information. Injectors would be the first thing I would have checked.
It is completely normal for a diesel to smoke black. But, if it just now started to smoke black and everything on the truck is stock, go get it checked out by a mechanic. You could have bad fuel injectors or a dirty air filter. Start with the simple stuff first. And, I'd say from it being a 2004 Powerstroke 6.0L, that your injectors are probably bad.
Take the diesel fuel filter off, and fill it with ATF, Automatic Transmission Fluid, Dextron II Run it! Simple as that
I'm confused. I can tell you how to change the tensioner on a 7.3L Powerstroke. The problem is Ford did not make a Powerstroke until 1994. Is your truck a 1992? What is the 8th digit in your VIN Number? If it's an F you have a Powerstroke. Let me know so I can help you with your question.
To change fuel injectors on a 2002 Honda Civic unclip them from the harness. The fuel rail will need to be removed and the injectors will be able to come off.
