Asumeing your refering to a automatic transmission, You replace the transmission with a 4 speed that will fit your engine mounts and frame, and make adjustments to the cables and carb linkage. If the stock transmission was computer controlled you will need to reprogram the ECU to acknowledge the existence of the additional gear. You will also have to install a new clutch and brake pedal. Unless you have a donor car this is going to be a very, very, expensive conversion.
