Could mean o2 sensor is lazy or it is actually running rich, but usually there is a seperate code for that one since 02s can adjust trim up to 25% in some cars. Anyone with a good scanner can graph your o2 and check your fuel trims to see which it is. Most likely the o2 or a bad check valve or regulator in fuel system. If your pressure is not sky high -it's the o2. Check pressure on the rail though-unregulated pumps can actually put out 100 pounds or more which could be very bad if you car only requires 60 for the time injectors are open.

If mixture gets rich enough compared to air it won't burnat all and if extra fuel is really going through your exhaust, it will backfire & eventually kill all your convertors.
