Depends on your head unit. If you have a factory CD controller (has a button that says CD on it), you will need a OEM changer unit and harness that goes from behind the rear seat to the changer.
If you want to install a new radio, there are many options.

In your center counsole, open it up and there is a black rectangle slide, slide that back and there it is, push the eject button and out pops your 6 disc CD changer.
is the whole unit a changer or is the changer in the trunk or something or under a seat?
Mine is in the trunk, its a 6 disc changer.

The 6 disk CD player was an option. Depending on what model you have on the 1995-97 Lincoln Continental, the changer is either in the arm rest area on the center console or behind the drink holder on the models with the column shifters.
It depends on the make/model of the CD Changer. Most aftermarket headunits have a compatible manufacture made changer. If you do not have a compatible unit you can purchase an FM Modulator for that changer. The modulator takes the signal and broadcasts it over a radio frequency.
On a 1999 Lincoln Navigator : If it is equipped with the six disc C/D changer , it is in the center console

You will need to go to a car audio store and buy a connector to tie into your car's wiring harness, and, the question you ask is too involved for a proper answer. You may want to go to a car parts store, or book store, and get a repair manual for your car. They cost about $16.00 Or, go to a Public Library.
