The respective door panels have to be removed - once they're off, simply remove the small mounting screws (5mm?) and disconnect the wiring harness. See "Related Questions" below for more about getting the panels off. ---- The front door panels must be removed to get to the speakers there.
For the rear speakers in 1995-1998 models, all of the plastic molding on each side needs to be removed. Start at the quarter window frame covers and work your way forward. It's a tough job but it can be done. Good luck. The first step is to remove the door panel.

With the panel off, the speaker is held on with 4 screws - very small, maybe a 7mm nut-driver. (been a long time since I did my 2001 rear speaker)
Disconnect the wire harness and swap in the new speaker.
FYI - I was desperate and bought a cheapie from a local 'big-box' retailer - given time I would have ordered it from Crutchfield.com! Their site will tell you exactly what sizes are needed, and likely to include the wire harness connector so you DO NOT have to cut the factory wiring.

The size of the front speaker are 5'x4'. You can get adaptor plates to fit 5" speakers. Premium audio also has tweeters. They are pop riveted to the a pillar which is more difficult to replace.
All you have to do is by new speakers. Most speakers connect to the component with any plugs. If the component isn't damaged, then just go and price some new speakers.
