1. Pop off the triangular piece that is just cosmetic on the upper flat part of the mirror where it is attached to the van. It has 3 tabs at the bottom, so pull upward and outward at the top. It is important to disconnect your vehicle battery for safety prior to starting the project.
2. Under this piece are two visible bolts which you will need a T-30 Torx driver to remove.
3. Open the passenger door all the way open and remove the 3rd bolt, which is exposed from the outside of the car.
4. Expose the wiring harness plug by doing the following:
a. Pull off the door weatherstripping along the top of the door and on the pillar (the part of the car to the left of the windshield) Do not completely remove the weatherstrip - simply pull it away enough to execute the next step.
b. Remove the plastic pillar cover: It has a button cover hiding a 10mm bolt. Remove the bolt and pull the cover toward the center of the car. It will pop off.
c. Lift up the plastic cover over the speaker on top of the dashboard. This piece goes all the way across the dashboard. Simply lift it up enough to get your hands underneath it. There is no need to free it from the passenger side. You may need a putty knife or similar device to pry the plastic cover from the foam dash cover, but it lifts fairly easily.
d. Follow the mirror wiring harness around the rear of the speaker (the side nearest the engine) and unplug it from the female connector at the right-rear of the speaker. There may be a wire clip at the left-rear of the speaker that you will need to get the wire out of.
5. Unplug the wire, remove the mirror, replace the mirror, reinstall everything

I recently replaced the passenger side view mirror and the information above was really helpful to me. There are a couple of things I want to add:
1. The info above is for drivers side mirror even though the title says passenger. The difference is that on passenger side plastic pillar cover wont simply come off without taking out the door handle first (the thing you're supposed to hold on to while sitting in the passenger seat). It has its own two little snap off plastic covers that come off, and then two nuts need to be removed to take the handle out. A small wrench will easily do the job.
2. The replacement part that I got is a heated power mirror, although the original was a regular power mirror; but either one works fine in the touring edition for 2005 T&C.

This information is also valid on a model 2006. On the 2007 T&C it helped me to lower the glove box and remove the vent in the dash as well as the plastic cover on the lower side of the dashboard near the door for better visibility. The power source (female) plug is up behind the speaker in a fixed position - tough place for large hands to reconnect the two plugs. It is not visible, so you have to feel your way around - give yourself plenty of time. the male plug of the electrical line coming from the mirror needs to have the clip facing up to get it in properly.
You didn't say what was wrong with the mirror, but if it is just a broken mirror you can find replacement mirrors which are cut to proper size and have double sided tape on back. Just peel of the backing from the new mirror and stick to the broken one being careful to allign before pressing for good. I did this four years ago and it still looks like new. This will work for any vehicle that they make the replacements for.
