Pretty easy actually; pry the top cap off the OEM shift knob with a small screwdriver or knife; underneath you will find a 1/2" nut; remove the nut with a socket and the knob will lift straight off.
Answer #2 I work at a Chrysler dealership and all the shifter knobs I have taken off especially 2008's are pressed. What you want to do is get some sort of adjustable wrench and put a towel around shifter knob so no damage is done. Slightly! Turn side to side while pulling up on the shifter. After it's loose pull straight up, it takes a little muscle. Try to do it by hand first using wrench as your last resort.
