You will need to remove the center console cover. The lightbulb will be attached to the inside of the center console. Push the lightbulb in and turn at the same time to remove it. Reverse the process to install the new lightbulb.
I doubt very much that the knob will come off of the automatic column shifter.
Column shift you can't. Need to replace the whole overdrive switch. Floor Shift, the bezel just pops off and you can access it.

It may be a broken or damage steering column. It may need to be replaced.
I'm not sure what a "shifter bulb" is, but if the shifter is actually broken, the most economical solution might be to swap in a salvage column, as I doubt any new parts are available at this point.
