Generally I would check it every 30k, and change it every 60k. They're not that expensive, and it wouldn't hurt to replace it a little early.

You should be able to find it in the valve cover or intake manifold. It's got an "L" (elbow") shape slip-on pipe connection, to which a rubber PCV hose attaches. The PCV valve sits in a rubber grommet, which fits into the port on the valve cover (or intake manifold). It's recommended that the grommet be changed at the same time as the PCV valve, since it'll most likely be old, dry, and brittle if it's never been changed.
