My 2000 did about the same thing. I had to have the shift solenoid in the tranny replaced. Cost was approx $550. Well I was told that I should clean all electrical connections to the trans, a signal may not be getting to the parts or controller, so I got some brake cleaner in a spray can and cleaned all connections, blew dry them with a air blower and connected everything back together, Took a test drive and within 5 minutes the CEL went off trans shifted fine and now 20 miles later no signs of trouble. Hope that fixed it.

Try changing transmission fluid and filter
Truck driving schools in the US like to teach you to upshift at 1500 RPMs and go back into gear at 1000 (this is with an unsynchronised transmission). Personally, I upshift at 1900 - when you go back into gear at 1500, it bogs the motor down, reducing the service life of the motor.
My assumption is that when "hold" is engaged the transmission will not upshift but instead stay in the selected gear
