The two main reasons could be either a leak or a hole. I'm not very knowledgeable when it comes to cars, so I'm probably wrong.
Usually means the over drive has been turned off. There should be a button usually on the gear shifter. On the column it is on the end of the shifter. Usually it resets when the engine has been turned off. This is something that you usually have to intentially do. If there is a problem with it then that could be also, but usually a check engine light comes on with it. Hope this helps.
