The O2 sensor on my 1993 B250 is located in exhaust pipe. Look under the van on the driver's side, under the driver's door. You should see an exhaust pipe that has what looks like a spark plug with wiring coming from the top. That is the O2 sensor. It is easy to change. I did mine two days ago and it took about 30 minutes to do. I spent most of my time trying to get the wiring harness connector worked loose, otherwise it is about a 10 minute job - if even that. baxman_dan@hotmail.com
A Dodge B250 Van does not have a mass airflow sensor.
It is screwed into the exhaust manifold.
Oxygen Sensor Circuit (Rich Exhaust). This may of may not be a defective O2 sensor. It may be something else that is effecting the O2 sensor. But considering the vehicle is 22 years old, I would suspect the O2 sensor needs replacing. I would replace the O2 sensor.

The 1987 Toyota Pickup came equipped in two 2.4L variants between turbocharged and naturally aspirated. This application has upstream sensors (Position 1), that is they are located on the exhaust manifold before the catalytic converter. The OE replacement O2 sensor is AC Delco #213119.
When those things on my 92 B250 did not work, I replaced the speed sensor located on the transmission tailshaft and that fixed the problems. Hope that helped! The O2 sensor can be found on the outside of the air cleaner housing. The O2 sensor will be attached to the back of the air cleaner housing.
