My cousin and I are putting his back on from slipping off as well. It seemed to happen to him when he drove through a large puddle and the water allowed slippage of the belt while running. If I had to guess this happens more often than most realize, probably due to pour design of undercarriage splash guards and such. There is quite a few posts in forums about the ZX2 Escort tensioner pulleys failing prematurely as well. It could've been a combination of both in our case or just dumb luck. Unless it happens again, we'll never know.

Release the pressure on the belt tensioner and belt should slide off pulleys
slacken tensioner with a long wrench, then slip belt off. Replace opposite way. Takes about 15 minutes.
To remove the serpentine belt on a 2003 Ford Excursion, first locate the belt routing diagram, usually found on a sticker under the hood. Use a wrench or a ratchet to rotate the tensioner pulley counterclockwise to relieve tension on the belt. Carefully slip the belt off the pulleys, taking note of its routing for reinstallation. Once removed, inspect the belt for any signs of wear before replacing it with a new one.
First you will need a special serpentine belt tool since you will never be able to fit a regular wrench in the idler tension pulley. Once you have this tool you heave on the pulley enough to release tension from the belt and slip it off the pulleys. Make sure you note how the new belt weaves through the pulleys first, and then reinstall it.

I had the same problem, so I took the belt off and washed it in soapy water. I then took a wet, soapy cloth and wiped off all of the pulleys. The squeal in my Aerostar was caused by a leaking power steering pump, which leaked fluid on the belt, causing it to slip and squeal.
a bad pulley or misaligned driven component. If one of the pulleys in the system is tilted it will drive the belt off in that direction. the belt has stretched and needs replacing
There should be at least one (or more) idlers. You can pry against the spring tension to move it/them & slip the belt off.
