The 25th Amendment to the United States Constitution, ratified in 1967, stipulates that if the President of the United States dies, resigns, or is otherwise unable to fulfill their duties, the Vice President of the United States shall assume the powers and duties of the presidency. The amendment also specifies that if the Vice President is unable to assume the office, the President's duties will pass to the Speaker of the House.
One example I have seen during my adult life is when the President is put under the effects of anesthesia for an operation, like after Ronald Reagan was shot. A more complete answer is any time the President is unable to fulfill the duties of the Presidency.
The Speaker of the House would become president if the president and VP were incapacitated at the same time.

If the President of the US cannot fulfill his or her term due to things like death, illness - physical or mental, disease or other factor, the President can resign from office whereupon the Vice President becomes President. Also, Amendment XXV (25) to the Constitution provides for circumstances where the Vice President becomes the Acting President. There is no such Ammendment and would almost never happen in today's politics as you would want someone to be on the "same page" if you were unable to fulfill the duties as president
Presidential succession, including the role of the Vice President, is primarily governed by the Presidential Succession Act of 1947 and the 25th Amendment to the U.S. Constitution. The Vice President is first in line to succeed the President if they are unable to fulfill their duties. The order of succession continues with the Speaker of the House, President pro tempore of the Senate, and then the Cabinet members, starting with the Secretary of State. Congress has the authority to modify the succession order through legislation.
