There are a number of threats to human existence, but as far as complete extinction a couple come to mind -- all out Nuclear War making the planed uninhabitable, or a massive asteroid collision. Conceivably a super-volcano could blanket the Earth blocking out the Sun, causing all complex life to die.
The event most likely to push us into a primitive existence is global warming which in turn will cause world hunger/starvation. Unless the atmosphere becomes to hot to support human life, some people will probably survive but their existence will be more like primitive man.
Another possibility is a plague or parasite that medicine cannot deal with. It seems unlikely it could annihilate all humans but might create a situation like the one mentioned above caused by global warming.
Though it seems unlikely we can exist that long, but eventually the Sun will explode and end all life in out solar system. If we haven't found a distant hospitable planet to move to it end human life.
