The organs that are in the left iliac region include distal portion of the descending colon, usually the sigmoid colon, and portions of the small intestine.

Well, honey, in the left iliac region, you've got the sigmoid colon, left ureter, left ovary and fallopian tube (if you're a lady), and maybe a bit of the left kidney hanging out nearby. Just a little neighborhood of organs chilling on the left side of your lower abdomen.
Internal Iliac
The appendix lies in the right iliac fossa. Not on the left lower side of the abdomen. Unless you have reverse position of the abdominal organs. So your pain is not likely to be of the appendicitis. Your physician will diagnose it. He may go for ultrasound examination of the abdomen.
Upper left
The iliac fossa region is located in the pelvic bone area, specifically on the inside surface of the ilium bone, which is one of the three bones that make up the hip bone. It is a shallow, bowl-shaped depression that serves as an attachment site for muscles and ligaments.
The right iliac fossa, located in the lower right quadrant of the abdomen, contains several important organs. Primarily, it houses the appendix, which is a small pouch connected to the large intestine. Additionally, portions of the cecum and the terminal ileum of the small intestine are also found in this area. In some cases, the right ovary and fallopian tube in females may also be located here.
