Merlin didn't have a last name. He was merely called: Merlin, or Myrddin, or Emrys. Surnames in the modern sense didn't exist at the time Merlin would have lived. The Romans had an elaborate naming system with Praenomen, Nomen and Cognomen for the men. But Rome wasn't much of an influence in Britain after around 410, so that naming system would have died out.
