This is called paraphimosis and is a potential medical emergency - it can result in difficulty urinating and a host of secondary conditions including kidney trouble.

Treatment starts conservative - a veterinarian will typically use a hypertonic solution to reduce the swelling (edema) in the exposed penis then manually reduce it back into the preputial orifice. If this doesn't work, the dog will need surgery to remove the exposed portion of the penis.
