The Roanoke colony failed once. It was the first English settlement in North America established in 1587, but by the time the relief expedition arrived in 1590, the entire colony had vanished with no trace of the settlers.
It is actually a mystery as to what caused Roanoke to fail as a colony. The only reason historians know that is failed is based on some settlers' accounts of what occurred: A ship with settlers arrived at Roanoke, only to discover that the colony was completely abandoned. The only clue that they found was the word "Croatoan," carved into a tree, but they were unable to figure out what the word meant.
