NO you don't, but you do get a choice of a Kanto starter and a Hoenn starter. To do so, you have to beat the elite four, get the Kanto badges and beat trainer red on mount silver. After you have done so proceed to talk to professor oak and he will give the Kanto starter of your choice, and to get the Hoenn starter as well as releasing Latias/Latios (depending on your version) to roam in Kanto just as Entei and Raikou did in Johto,

Actually, to get the Hoenn Starter you must go to the Silph Co. Then Steven will be there and talk to him. Then you will have a choice of a Green Stone (Treeko), Blue Stone (Mudkip), and a Red Stone (Torchic).

No you cannot get a Sinnoh starter in Pokemon heart gold. For starter Pokemon in this game, you can choose chicorita, cyndaquil, and totodile. But if you desperately want to get sinnoh starters, you're gonna have to trade from the older versions platinum, diamond and pearl.
