One pixel at a time, literally. Be sure to not use the same color of pixels repetitively, too. If you look at your skin, close up, you will see that it is full of color discrepancies, pores, shadows, hair, freckles, tans, scars, etc. So each tiny pixel needs to reflect that similarly. I still am not sure that you will be able to make it look realistic, but it's worth a try.

While the background was painted in a way that would give the image depth and may not have existed, the subject did; and as it is a portrait of her, I would say it is meant to be realistic in it presentation of her image. However, in the arts realistic or realism means something slightly different than a true image of the subject. Rather, it means the subject of the image is as it would be in everyday life. So the Mona Lisa being a planned portrait would not be realism or realistic.
