If you want to paint do you have to start out on canvas?

Yes, however, in many cases the graphite can and will either show through or lift to the surface, so be certain to use a very hard pencil, and the minimum amount of lines you need as reference.

There are different systems... often, first a glue or binder is applied, to make the canvas non-absorbent, also to make the canvas shrink-to-fit. Then a primer paint: white gesso (acrylic gesso, or home-made gesso). It can be a colored gesso as well (if you make your own).

A canvas primer needs to be flexible, so it won't crack in time.

Most canvases come pretreated. if not some art stores will help you out. Currie's is amazing with supplies. They have pretreated canvases. If it is not pretreated yes you will want to treat it.

It's not necessary, by putting on a background paint or something like that, basically treating the canvas first, will achieve a different effect.

The canvas is usually tacked onto a 'stretcher' and is placed an easel.

No. You can start out on many less expensive "grounds", depending on the kind of paint you intend to use. Ask at you local art or craft store for advise.
