Often, but not always. "Matriarch" is formed of two words: "mater", meaning mother, and "archon" meaning leader. A matriarch is therefore a mother who is the leader of the family. If the mother is not a leader of the family she is not a matriarch. If there are no parents, the leader of the family may be the eldest sister, but although she is the oldest female, she is not a matriarch because she is not a mother.

She is the most powerful female of the older generation which, in most cases does mean the oldest female. But she could be the widow of the patriarch even though she there is an older sister-in-law.
