Leaking lysosomes (actually, leaking phagosomes, but they can be considered as one for now) release hydrolytic enzymes into the cell; these enzymes are capable of digesting a variety of cellular substances. The unintentional digestion causes cell damage, leading to inflammation etc.

This is only a hypothesis, and has yet to be proven conclusively.
