Currently there are no State or Federal License requirements to do tattooing in the State of Utah. From ages ago, tattoo inks are not regulated by the FDA nor by the State of Utah. There are two other licenses required; one is from the Health Department which costs $75/ yr. It requires that a tattooing establishment follow certain rules in regards to cross-contamination prevention, i.e., the use of a Auto-clave for re-usable machine parts, floor must be non-poris and a restroom must be available, plus all the other things required for any business, heating, lighting, etc. The State Health department does not give a test for these things however to enforce compliance, unannounced visits are common. The second license required to do tattooing in the State of Utah is a license to work - there are several types that can be had: DBA, LLC, S-CORP etc. No other license or testing is required. There is a stiff penalty for tattooing on a minor $750 - $1000.
