Well, a teardrop tattoo on one's face, like where a real tear would drip, is usually adorning the face of a killer. Alot of inmates get teardrops, each standing for a person they have killed. The heart I'm not so sure about.
The first tattoo dates to over 5000 years ago, there is no way of knowing the first person.
