It depends on the person.
In some scenarios it could symbolize infectious or possibly the person has an infectious disease. Some say it symbolizes "anti system." I know of someone who says it means "I'm hazardous to your health."
Let's realize that other people will make decisions regarding their lives and you can extrapolate and calculate and revise and refine and yet at the end of the day, when a person has a tattoo on their body, it is just a tattoo! You can draw all sorts of inferences regarding the motivation, the context, the person, etc., but sometimes, when you don't know them well, you really can't even begin to comprehend the situation.
That's not to say that tattoos born by groups of people do not have any sort of relationship to any sort of deeper culture. They might and they might not. I've met many people with tattoos that have all sorts of non-existent conscious meaning, in the sense that the person might have just wanted to put a biohazard on their arm.
Or, maybe they do have HIV. That could be true. There are definitely groups of people who deliberately seek out HIV infection.
