Heart failure is when the heart cannot pump the blood out as fast as it is comming back in. As a result pressure builds in the veins and fluid leaks into the surrounding tissues.

If the left heart fails the fluid goes into the lungs and causes shortness of breath and eventually right heart failure.

If the right heart is failing, either due to left heart failure or on its own, you get swelling in the legs and abdomen.
