The sternum (breastbone) move up and out slightly, moving the ribs slightly with it. This creates a pressure gradient where there is very low pressure in the lungs and higher pressure out. As a result of this gradient, air is almost sucked into the lungs, like air filling a vacuum space. As the diaphragm contracts and gravity pulls the ribs and sternum back down, the air is forced out of the lungs.

Reflections off the glass on the inside are there but in the car is so dark the reflections are darker than the view through the window. The view is brighter than the reflections. Of course if it is dark out side those inside can not see out. Turn the lights on in the car and the situation is reversed, people can see in, but those inside an can not see out.
